Becoming a Patient
In an effort to better serve you we’ve included the patient intake forms for download below. If you wish, you may fill out and sign each of them before your first appointment. Feel free to call with any questions.
Please allow approximately an hour and a half for your first visit and one hour for subsequent visits. Wear something comfortable and loose fitting.
Payment in the form of Visa, MasterCard, check or cash, are expected at the time services are rendered. Acupuncture and related treatments are reimbursable expenses in the state of California. For more information on insurance coverage, click HERE.
